Personal Data Collection,
Processing, and Consent Form

I freely and voluntarily consent to IANsmart Energy Corp. (“IANsmart”) collecting, obtaining, receiving, accessing, using, storing, and processing my personal information and supporting documents, whether in physical or electronic form, for purposes directly related to my application and proposed solar installation project, including assessment, verification, preparation of quotations and proposals, evaluation of applicable financing requirements, and processing of the proposed transaction.

IANsmart may collect and process the following personal information and supporting documents related to the application, namely:

• Personal identification documents and government-issued IDs;
• Contact information, including full name, address, telephone number, and email address;
• Proof of billing and electricity consumption records;
• Property ownership, occupancy, or authorization documents;
• Financial and income-related documents, when required for financing evaluation;
• Photographs, videos, and other information related to the proposed installation site; and
• Other documents reasonably necessary for the assessment, quotation, verification, and processing of my application.

Only information necessary and relevant to the declared purposes shall be collected and processed.

Only authorized personnel, agents, representatives, and partners of IANsmart, including its designated Project Approver, shall access, handle, and process the personal information and supporting documents solely for purposes directly related to the evaluation, verification, processing, and approval of my proposed solar installation project.

IANsmart shall implement appropriate organizational, physical, and technical safeguards to protect my personal information and supporting documents against unauthorized access, use, disclosure, alteration, loss, or misuse.

IANsmart may disclose my personal information and supporting documents to authorized financing institutions and service providers to evaluate and process my proposed transaction, subject to applicable data privacy laws and appropriate safeguards. I voluntarily authorize the sharing of my relevant personal information and supporting documents with the financial institution and service providers solely for that purpose.

In the event that I decide to withdraw, cancel, or discontinue my application for the proposed solar installation project, I understand that I may communicate my withdrawal to IAN Smart Energy Corp.

Upon receipt and processing of my withdrawal request, IAN Smart Energy Corp. assures me that the personal documents and records I previously submitted and that remain in its possession will be securely disposed of, subject to any applicable legal retention requirements.

Such disposal will include:

• Physical Documents: Secure shredding or other appropriate disposal methods to prevent unauthorized access, retrieval, reconstruction, or further processing.
• Electronic Records: Secure deletion of electronic files and records to prevent unauthorized access, retrieval, reconstruction, or further processing.

The company will implement appropriate disposal measures to safeguard my personal information and prevent unauthorized use or disclosure.

IAN Smart Energy Corp. respects my right to privacy and is committed to protecting my personal information in accordance with Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012, and its Implementing Rules and Regulations.

I understand that I have the right to be informed about the collection and processing of my personal information, to access and request corrections to my information, and to exercise other applicable rights under the Data Privacy Act.

By signing below or providing my affirmative electronic acknowledgment, I confirm that:

• I have fully read and understood this Form, including the purposes for which my personal information and supporting documents will be collected, processed, and used.
• I voluntarily consent to the collection and processing of my personal information for the purposes stated in this Form, where consent is required by law. I also understand my rights under the Data Privacy Act of 2012, including the right to withdraw consent, subject to applicable laws.
